Abstract: Background: Polycystic ovary syndrome (PCOS) is a prevalent endocrine disorder affecting women of reproductive age and has significant long-term implications for health. Methods: This review focuses on elucidating the causes, pathophysiology and diagnosis of PCOS. Initially, we have provided a comprehensive overview of the current understanding of PCOS causes and pathology. Subsequently, we delved into the intricate details of causes that play a role in PCOS pathogenesis. Finally, we present insights into potential avenues for future research. Main findings: The pathophysiology of PCOS is multifaceted, involving the interplay between reproductive dysfunction and metabolic disorders. Hyperandrogenism and insulin resistance mutually exacerbate each other throughout the progression of PCOS, which is further influenced by disruptions in the hypothalamus-pituitary-ovarian axis. While PCOS has a significant genetic component, certain environmental factors play a more substantial role in driving its pathophysiology. Conclusion: Based on our current understanding, three key areas are proposed as potential subjects for future research. These endeavors aim to develop therapeutic and preventive strategies, as well as provide comprehensive lifelong management, including preconception care, for individuals affected by PCOS.
