Abstract

Image in paediatric Definition and Types of Macroglossia: Macroglossia is a clinical disorder in which the tongue is larger than normal. Enlargement can be true as seen in vascular malformations or it could be due to muscular enlargement {1}The prevalence of congenital macroglossia is less than 5 per 100,000 birth. [2}Macroglossia, also known as giant tongue or enlarged tongue, is a rare condition that affects children more than adults. It is often associated with other conditions, such as Beckwith-Wiedemann syndrome, Down syndrome, congenital hypothyroidism, and certain tumours. Symptoms in children include sleep apnea, respiratory distress, drooling, swallowing difficulties, and speech problems. If left untreated, macroglossia can lead to long-term complications, such as anterior open bite deformity, frequent upper respiratory tract infections, and failure to thrive. Treatment depends on the underlying cause. {3}Various causes include acromegaly, Beckwith-Wiedemann syndrome, congenital hypothyroidism, Down syndrome, and lymphangioma or haemangioma. It may cause significant symptoms in children such as sleep apnea, respiratory distress, drooling of saliva, difficulty in swallowing and dysarthria. Long-standing macroglossia leads to an anterior open bite deformity, increased incidence of upper respiratory tract infections and failure to thrive.{4}Macroglossia , sometimes called giant tongue or enlarged tongue,is a rare condition that typically affects more children than adults.Most people have macroglossia because they have other conditions,such as Beckwith-Wiedremann syndrome or Down syndrome .Treatment for macroglossia varies depending on the underlying cause.{5} In some instances,children who have macroglossia outgrow the condition as the bones in their face grow and their mouth have room for their tongues.When healthcare providers treat macroglossia ,they start with diagnosing and treating the underlying condition ,then treating macroglossia.{7}Macroglossia treatment may include ,Medication such as corticosteroids for swelling,Orthodontic treatment,Surgery,{8}About 10% of people treated for macroglossia have surgery to reduce the sizeof their tongues.We describe a patient who was 10 month old and visited the paediatric department. with complaints of swelling over the tongue at birth ,mother notted a swelling over the tongue which was initially small in size,which has now progressed to present size no complaining difficulty in taking feeds,no difficulty in breathing ,no history of fever ,cough,no excessive cry no bleeding from site .vital sign heart rate is 90/min,respiration rate 22breath /min,SPO2-98,A large soft cystic swelling present in up to 5cm seen over tongue.{9}. She has no history of trauma and constitutional symptoms. Investigation done like ultrasonography ,x-ray can be used to determine the size of the tongue test may be include gene analysis,biopsy,or fine needle aspiration.and in some cases surgery is required reductive glossectomy{10} Key words : Macroglossia, Tongue reduction, Partial glossectomy
