Cerebral Chaos: Arterial Ischemic Stroke and Cerebral Venous Thrombosis in the Setting of Diabetic Ketoacidosis

Neurological deterioration in diabetic ketoacidosis (DKA) can arise from electrolyte imbalances or cerebral edema, but focal neurological deficits may indicate underlying complications such as stroke or central venous sinus thrombosis, particularly in the context of hyperglycemia. We present a case of a woman in her 40s with newly diagnosed type II diabetes, who exhibited altered consciousness and significant neurological deficits during DKA. Imaging revealed multiple ischemic strokes and extensive cerebral venous thrombosis, compounded by a severe protein C and S deficiency. Despite aggressive treatment, the patient succumbed, underscoring the need to consider thrombotic complications in DKA, particularly in patients with diabetes. This case illustrates the complex interplay of metabolic and coagulopathic factors in DKA and the importance of vigilant assessment for vascular complications.
