Purulent pericarditis is a rare but life-threatening condition that, despite modern antibiotic therapy, remains associated with high mortality. It typically arises from direct pulmonary spread or hematogenous seeding, with known risk factors including pre-existing pericardial effusion, immunosuppression, and prior cardiac surgery. However, cases can occur in patients without identifiable risk factors, emphasizing its unpredictable nature. Early diagnosis using echocardiography and prompt intervention with pericardiocentesis, microbiological confirmation, and targeted antibiotic therapy are critical to improving outcomes. This case underscores the importance of maintaining clinical vigilance for purulent pericarditis, even in atypical presentations.
