Stab wounds to the spine, though rare, pose a significant risk to the spinal cord despite the protective role of the vertebral column. This case describes a 31-year-old man who presented with a stab wound to the lumbar spine but exhibited no neurological deficits. Initial imaging revealed a metallic foreign body near the second lumbar vertebra. The knife was successfully removed under general anesthesia without complications, and the patient remained neurologically intact. Lumbar stab injuries can result in severe outcomes such as neurological deficits or epidural hematoma, yet management remains controversial in cases without initial symptoms. This case highlights the importance of individualized treatment, emphasizing that timely intervention and careful monitoring are crucial for a favorable prognosis, particularly when no neurological symptoms are present.
