Nitric oxide (NO) is a key paracrine mediator and plays a crucial role in regulating several female reproductive processes, including ovulation, implantation, pregnancy maintenance, labor, and delivery. During follicular development, NO levels rise, but decrease immediately after ovulation. In the endometrium, NO influences important functions such as endometrial receptivity, implantation, and menstruation. It has been suggested that NO donors could aid in enhancing fertility, while NO inhibitors might be explored for contraceptive purposes. Regarding uterine contractility, the production of NO diminishes, leading to stronger contractions during labor. Clinical research has shown that NO donors can be effective as tocolytics. For cervical ripening, NO levels in the cervix are low throughout most of pregnancy but increase as term approaches, and NO donors have been found to be effective and safe agents for cervical ripening. NO also acts as a powerful vasodilator in blood vessels and inhibits platelet aggregation. Within the uterus, NO generation suppresses uterine contractility. The modulation of NO production and its effects on the uterus are influenced by gestation, parturition, steroid hormones, and prostaglandins, supporting the theory that NO helps maintain uterine quiescence during pregnancy.
