Abstract

Background: Aging is associated with progressive decline in cognitive and motor functions, which significantly affects independence and quality of life in older adults. Cognitive impairment and balance dysfunction are major contributors to increased fall risk in the geriatric population. Understanding the relationship between cognitive domains and balance is essential for developing effective fall prevention and rehabilitation strategies. Methodology: A cross-sectional observational study was conducted among 51 older adults aged 65–80 years residing in geriatric care homes in Nashik, Maharashtra. Participants were selected using purposive sampling based on predefined inclusion and exclusion criteria. Cognitive function was assessed using the Montreal Cognitive Assessment (MoCA), and balance was evaluated using the Fullerton Advanced Balance (FAB) Scale. Data were analyzed using SPSS version 26, with descriptive statistics and Spearman’s correlation test applied to determine associations between cognitive domains and balance performance. Results: The mean MoCA score was 21.31±3.30, indicating overall cognitive impairment, while the mean FAB score was 21.60±4.15, suggesting a high risk of falls. A statistically significant positive correlation was found between total MoCA score and FAB score (ρ=0.327, p=0.019). Among the cognitive domains, only attention showed a significant positive correlation with balance (ρ=0.351, p=0.012). Other domains such as visuospatial/executive function, language, abstraction, and orientation did not show significant associations, although delayed recall demonstrated a near-significant trend. Conclusion: The study concludes that cognitive function, particularly attention, plays a crucial role in maintaining balance and postural control in older adults. Impairment in attention significantly increases the risk of balance dysfunction and falls. These findings highlight the importance of incorporating cognitive assessment and targeted cognitive interventions, especially attention training, in fall prevention and geriatric rehabilitation programs.
