Abstract
Spondyloarthritis is a group of chronic inflammatory joint diseases that mainly affect the spine, sacroiliac joints, and sometimes peripheral joints. They are commonly associated with the gene HLA-B27. Main features seen in spondyloarthritis are Axial involvement and Peripheral arthritis. Axial involvement includes Chronic inflammatory low back pain, Sacroiliac joint inflammation (sacroiliitis) and Spinal stiffness and Peripheral arthritis includes usually asymmetrical lower limb joint (knee, ankle, hip) joint involvement. This case report presents the management of a 23-year-old male diagnosed with peripheral spondyloarthritis, with HLAB27 positive, increase level of CRP and ESR clinically shows severe pain and swelling in B/L knee joints, B/L ankle joints, and right elbow joint along with restricted range of flexion and extension of limbs; he has been unable to walk for 4 months, but the patient has no back pain. The patient was treated on the line of treatment of Pravaddha Aamvata with a combination of Ayurvedic therapies, including the Panchkarma procedure Vaitaran basti(enema with medicated oil) and Shaman Aushadh. Remarkable result was seen in the pain and swelling of the ankle joint with flexion and extension of the knee joint, the patient was able to walk without any support after treatment and showed marked improvement in CRP and ESR levels.While this case study provides optimistic findings, further research through more case reports and planned clinical studies is necessary to prove the efficacy of Ayurvedic treatments in managing HLAB27 positive arthritis.
