Abstract: BACKGROUND:Isoniazid-resistant TB (HR-TB) is resistance to isoniazid and susceptibility to rifampicin has been confirmed. It estimates 7% among new tb cases and 8%–11% among previously treated tb cases. KatG and InhA mutations account for 70-80 % of isoniazid resistant TB. AIMS AND OBJECTIVES:To study the clinical , radiological and microbiological case typing of isoniazid mono resistant TB cases • To assess the efficacy of isoniazid mono regimen under current NTEP • To evaluate the pattern of drug resistance in treatment failure cases MATERIALS AND METHODS: Hospital based prospective observational study done on 100 patients from December 2020 to january 2022 with complete clinical , radiological and microbiological history. RESULTS: HR-TB was more common in males aged between 45 -60 years with KatG as most common gene mutation. Cavitatory lesion was found in almost 62% of the patients with 84% cure rate by H mono regimen CONCLUSION: In conclusion, patients with isoniazid mono-resistant tuberculosis who took rifampicin for the entire duration had a higher rate of treatment success. Patients who are unable to tolerate rifampicin may benefit from the addition of newer fluoroquinolones to their therapy regimen..Fluoroquinolone resistance augmentation is most frequent in treatment failure cases. Sputum non conversion at the end of 3 months is a warning state of impending FQ resistance Patients with cavitary lesions on CXR had a higher relapse rate and is associated with poor outcome even after extended H mono regimen.
