Abstract: This paper critically analyzes the National Health Insurance Scheme (NHIS) in Nigeria, focusing on its establishment to address healthcare challenges, particularly in the face of increasing costs and limited access. Despite its inception in 2005, the NHIS predominantly covers only 5% of the formal sector, leaving out the majority of the population, including the informal sector, poor, and vulnerable groups. This study emphasizes the urgent need for service improvement through a change in the legal framework and operational approach of the NHIS to encompass a broader demographic. The proposed change process involves a strategic and collaborative effort among government, legislative bodies, healthcare professionals, civil society, and other stakeholders. Drawing on Kotter's model, the study advocates for a sense of urgency, the creation of a presidential steering committee, clear vision communication, stakeholder engagement, and capacity building. The implementation of a short-term pilot scheme is suggested as a tangible demonstration of the envisioned improvements, followed by consolidation and institutionalization of the new approach. The implications of the change process are carefully examined, highlighting both strengths and weaknesses of using Kotter's model in a public sector context. While the model offers a predictable and manageable approach, its applicability to governmental organizations, especially those marked by bureaucracy and political intricacies, raises concerns. The study concludes by underscoring the significance of addressing these challenges to achieve a repositioned NHIS that ensures universal health coverage and equitable access to healthcare services for all Nigerians.
