Abstract: This study delves into the intricate diagnostic and therapeutic challenges in a 58-year-old male with Wallenberg syndrome, initiated by medullary infarction. The patient's clinical presentation, marked by rightward swaying, facial asymmetry(Mild left-sided Ptosis of eyelids), and cranial nerve irregularities, aligns with classical Wallenberg syndrome manifestations, prompting a detailed diagnostic exploration. The patient's admission was triggered by symptoms like headache and rightward swaying, Past medical history revealed complaints of diabetic retinopathy, and prolonged diabetes mellitus for a decade. Neurological deficits identified during examination, along with MRI findings, confirmed a left-sided medullary infarct, unveiling changes in the frontoparietal lobes indicative of small vessel ischemic disease with chronic lacunar infarcts. The medulla oblongata's vital role underscores the diverse symptoms of Wallenberg syndrome. Recognition of small vessel ischemic disease and chronic lacunar infarcts emphasizes the need for a multidisciplinary approach to address underlying vascular risk factors. Upon diagnosis, a comprehensive strategy integrated supportive measures and targeted pharmacotherapy (intravenous fluids, antiplatelets, antiepileptics, and antidiabetic medications). The patient's stable hemodynamics at discharge highlights the success of this holistic approach. With no definitive cure, Wallenberg syndrome prognosis varies. This case demonstrates diverse outcomes, with stability achieved post-pharmacological intervention. Positive two-month follow-up assessments underscore potential improvement and rehabilitation, emphasizing the need for thorough care. In conclusion, Wallenberg syndrome's multidimensional nature necessitates diligent diagnostics and a comprehensive, multidisciplinary management strategy. Recognizing associated vascular issues is crucial for addressing risks, preventing complications, and enhancing patient outcomes. This study provides valuable insights into Wallenberg syndrome complexities, emphasizing the ongoing need for thorough care and management.
