Abstract: Acute Coronary Syndrome (ACS) presents a critical challenge in cardiovascular medicine, being the leading cause of morbidity and mortality globally. Prompt and accurate management of chest pain, a hallmark symptom of ACS, is imperative for both symptom relief and disease progression assessment. This systematic review examines the balance between providing immediate symptom relief and evaluating the evolving disease status in ACS. Methodologically, a comprehensive literature search was conducted using PubMed, MEDLINE, and the Cochrane Library, focusing on peer-reviewed English-language publications from 2000 to 2024. Both randomized controlled trials (RCTs) and observational studies were included to provide a holistic understanding of ACS chest pain management. Current approaches in ACS chest pain management primarily target rapid symptom relief through pharmacological interventions such as analgesics, opioids, and nitrates. However, the efficacy of these treatments in the long term remains underexplored. Diagnostic advancements, including high-sensitivity troponin tests and imaging modalities, have revolutionized ACS management, emphasizing the importance of revascularization strategies. A critical analysis reveals a gap in literature regarding the long-term consequences of pain management strategies, the absence of personalized pain control approaches, and the exploration of innovative therapeutic options. Additionally, there is a need for further research to elucidate the impact of pain management on overall ACS prognosis. In conclusion, while significant progress has been made in ACS pain management, there are still challenges to overcome. Future research should focus on long-term outcomes, personalized treatment plans, and the development of novel therapies to enhance the quality of care for ACS patients. This review underscores the importance of striking a balance between immediate symptom relief and comprehensive disease management in ACS.
